Step 1: Application

Step 2: Initial Contact
- Potential candidates will receive a call from the Surpassion Healthcare Services, INC. administrator for a phone interview.

Step 3: Written Test
- Qualified applicants must take and pass the Health Aide written test with a score of at least 80%.

Step 4: Skilled Assessment
- Exam passers will undergo a skilled assessment with Dr. Ortega. More details can be found on Career Nursing Academy.

Step 5: Employment Application
- Those who pass the skill assessment must complete an employment application and submit the required documentation via upload, fax, email, or mail.

Step 6: Pre-Hire Checklist
- Complete the pre-hire checklist, which includes:
- Statement of Employability
- Request for Criminal History Form
- Sworn Statement or Affirmation for child-placing agencies
- Signature Attestation
- VA Social Service Central Registry
- I-9 Form
- Two Reference Check forms

Step 7: Pre-Hire Documents Submission
- Submit the completed pre-hire documents, including:
- Direct Support Professional Test (DSP)
- ACHC Test
- Adult and Child Mandated Reporter Test (Mandated Reporter)

Step 8: Employment Packet
- Applicants who pass the pre-hire tests will receive the remaining employment packet and new hire form to fill out and submit.

- E-Verify Check
- Social Security Number Check

Step 10: Training and Onboarding
